To my knowledge, he went from "probable" for Week 2 to still being out with no real clarity on the severity of his groin.
Since he got the injury last Friday working out on his own, I suspect the trainer can't be blamed for this one.

Agreed. There are usually only two ways you pull a hammy in this situation. One is that you didn't stretch and loosen up. The other is you were out of shape to begin with. Almost sounds like he just started working out a week before he was to come back. But who knows....?
And to make it clear, the CS is charged with ensuring that players warm up/stretch properly, and that they stay in shape. This isn't to absolve Herndon in any way, but the CS bears some responsibility too.

Totally agree, really dumb rule for a lot of reasons. Basically the league is saying the guys who need the structure of the team the most are on their own if they can't live up to their responsibility to the team the league and basic community standards. It's ass backwards.
They are not allowed to have contact with suspended players. Certainly not to the point where he could monitor Herndon’s exercise routine. It was cooler Friday than it had been. He probably had trouble loosening up before going into his route running.
